Whether you need a hand with the house or daily clinical care, supports are bundled into one coordinated plan managed by one local team.
Clinical care
Nursing care, wound care and management, continence support, physiotherapy, occupational therapy, podiatry, dietetics and nutrition support.
Independence support
Personal care including showering, bathing, dressing, grooming, oral care and mobility and transfer assistance.
Everyday living
Domestic assistance and light housekeeping, meal preparation, social support and companionship, transport and shopping assistance.
Assistive technology and home modifications
Equipment, aids and home safety modifications such as rails, ramps and shower chairs to support safe, independent living.
Restorative care pathway
Short, intensive allied health support to help you regain function and independence after a decline in health.
End-of-life pathway

Hospital-to-home
A safe, coordinated transition home from hospital
Leaving hospital is where recovery most often goes wrong. We work with discharge teams, social workers, case managers and GPs to have care, equipment and a safe home ready before you walk back through your front door.
Transition support can include discharge planning, home safety assessment and set-up, transport home, immediate post-discharge nursing, wound care and medication support, short term restorative and reablement therapy, and equipment hire such as hospital beds and mobility aids.
When short-term transition support ends, we hand over seamlessly into ongoing home care or, for younger clients, an NDIS support package — with the same workers wherever possible.
