Why it matters
Chronic wounds heal on the right plan, not the right luck
A wound that has not improved in four weeks is not simply slow — it usually means something in the plan needs to change. Under-treated oedema, a dressing that is wrong for the exudate level, unmanaged pressure, poor nutrition, undiagnosed arterial disease or low-grade infection are the usual culprits, and each has a different fix.
Our nurses work from measurement rather than impression. Wound dimensions, tissue type and exudate are recorded at every visit, so stalled healing is visible early and escalated to your GP or a wound specialist instead of continuing for months on the same dressing.
Where wound care sits alongside other clinical needs, we combine it with post-hospital nursing supports and daily personal care supports so showering and dressing routines protect the wound rather than disrupt it.

Getting started
Home wound care in four steps
1. Call or send a referral
Call 1800 343 012 or have your GP, hospital or case manager send the wound referral.
2. Nurse assessment
A registered nurse assesses the wound at home and sets the dressing plan and visit frequency.
3. Scheduled dressings
Aseptic dressing changes at the agreed interval, with sterile equipment brought to every visit.
4. Review and report
Measured healing reviews, plan adjustments and clear reporting to your GP and family.
