What our community nurses provide

Every visit begins with an assessment and ends with documented notes, so your GP, allied health team and family stay informed.

  • Wound and pressure care

    Simple and complex wound dressings, surgical wounds, pressure injuries, skin integrity checks and prevention plans.

  • Medication management

    Webster pack management, administration, injections, insulin and medication reviews with your prescriber.

  • Catheter and stoma care

    Urinary catheter insertion, changes and troubleshooting, plus stoma care and continence assessment.

  • PEG and enteral feeding

    PEG site care, feed set-up and monitoring, and training for families and support workers.

  • Chronic condition monitoring

    Diabetes, respiratory, cardiac and epilepsy support with observations, early warning signs and escalation plans.

  • Nurse delegation and training

    Competency training for support workers, documented delegation and regular clinical review of every plan.

Why nurse-led matters

Clinical confidence in your own home

Many providers subcontract nursing. We do not. Because Loving Hands Care was started by registered nurses, clinical decisions are made in-house — assessments, care plans, worker competency and escalation all sit with the same team that visits you.

That continuity keeps people out of hospital. Our nurses notice early changes in a wound, a chest, an appetite or a mood, and act before it becomes an ambulance call. Notes are shared with your GP and support coordinator so nothing is repeated or missed.

Nursing pairs naturally with our in-home care, Supported Independent Living and aged care services, and veterans can access the same clinical team through DVA community nursing.

Getting started

How a nursing referral works

Referrals come from participants, families, support coordinators, GPs and hospital discharge teams.

  • 1. Referral or call

    Share the clinical need, funding source and any discharge or GP paperwork you already have.

  • 2. Nurse assessment

    A registered nurse assesses at home, confirms equipment and consumables and documents risks.

  • 3. Care plan and quote

    You receive a written care plan, visit schedule and quote against your NDIS, DVA or aged care funding.

  • 4. Visits and review

    Care begins with the same small team, and the plan is formally reviewed as your health changes.
