What we help with

Community access and transport support we provide

Anything that gets you out of the house safely — health appointments first, but social connection matters just as much.

  • Medical appointment transport

GP, specialist, hospital outpatients, pathology, dental, optometry, podiatry and allied health visits, with waiting time included.

  • Escort and advocacy

Your worker can sit in, take notes, confirm follow-ups and pass instructions to family so nothing is missed or forgotten.

  • Shopping and errands

Grocery shopping, chemist collections, bank, post office and paying bills — with help carrying and unpacking at home.

  • Social outings and clubs

Coffee with friends, church, RSL and social clubs, exercise or hydrotherapy groups, library visits and day trips.

  • Mobility assistance

Safe transfers in and out of the vehicle, plus walker, wheelchair and walking stick support from door to door.

  • Companionship

A friendly, consistent companion for the whole outing — reducing the isolation that so often follows losing a licence.

How it works

Booked around your appointment, not our roster

Tell us the date, time and address of your appointment and we roster a worker to arrive early enough to get you there without rushing. They help you lock up, assist into the vehicle, park and walk you in, then stay until you are ready to leave.

Regular commitments — a fortnightly specialist review, weekly shopping or a Thursday social club — can be set as a standing booking with the same worker, so you always know who is coming and when. Any change in mobility, confusion or fatigue during the outing is reported back for review.

Community access pairs naturally with personal care supports in the morning and domestic supports on the same day, so one visit covers several needs.

Who it suits

Appointment transport is a good fit if you

Losing the ability to drive is one of the biggest triggers for missed care and social isolation. This service is designed to remove that barrier.

  • No longer drive, or no longer feel confident driving
  • Find public transport difficult with a walker or wheelchair
  • Have frequent specialist, dialysis or allied health appointments
  • Need someone to remember what the doctor said
  • Have family who work and can't take time off for appointments
  • Want company for shopping, clubs or social outings

Call 1800 343 012 to arrange transport or set up a standing booking. You can also see the full range on our home care hub or read about hospital-to-home transition care if you are coming home from hospital soon.
