As a Support at Home provider, we deliver subsidised home care across three core service groups, plus two dedicated care pathways.

  • Clinical care

    Nursing care, wound care and management, continence support, physiotherapy, occupational therapy, podiatry, dietetics and nutrition support.

  • Personal care

    Showering, bathing, dressing, grooming and mobility assistance, delivered with dignity and consistency.

  • Everyday living

    Domestic assistance and light housekeeping, meal preparation, social support and companionship, transport and shopping assistance.

  • Restorative care pathway

    Short, intensive allied health support to help you regain function and independence after a decline in health.

  • End-of-life pathway

    Dedicated palliative support to help people remain at home in comfort, surrounded by family.

  • One coordinated plan

    All supports bundled into a single plan with one local team and one point of contact as needs change.

Hospital-to-home

Hospital-to-home transition care

We specialise in a safe, well-coordinated transition from hospital back home — reducing avoidable readmissions and giving families peace of mind.

  • Discharge planning support — liaising with hospital discharge teams, social workers and case managers to plan care before you leave hospital
  • Transition Care Program (TCP) support — up to 12 weeks of goal-directed nursing, physiotherapy, occupational therapy, podiatry, speech pathology and personal care
  • Home safety assessment and setup — checking the home is safe and fitted with any equipment or modifications needed before discharge
  • Transport home from hospital
  • Immediate post-discharge care — nursing, wound care, medication support and personal care in the days and weeks after discharge
  • Short-term restorative and reablement support to regain strength, confidence and daily living skills
  • Equipment hire and coordination — hospital beds, mobility aids and other equipment arranged for the home

Clinical follow-up at home is delivered by our community nursing team, and longer-term support continues through aged care at home.
