What our DVA nurses assess and monitor
One visit covers your whole clinical picture, not just the condition you were referred for.
Vital signs and observations
Blood pressure, pulse, temperature, respiration, oxygen saturation, weight and blood glucose where indicated.
Falls and mobility risk
Balance, gait, transfers, footwear, home hazards and equipment needs, with referral for therapy or aids.
Skin and pressure area checks
Full skin inspection, pressure injury risk scoring and early treatment before breakdown becomes a wound.
Medication review
A check of every medicine at home against your current prescriptions, with discrepancies raised with your GP.
Nutrition, hydration and wellbeing
Weight trends, appetite, fluid intake, mood, sleep, memory and social isolation screening.
Care plan and GP reporting
A documented plan with monitoring targets and escalation triggers, shared with your treating team.
Why it matters
Trends tell the story a single reading cannot
One blood pressure reading at a GP appointment is a snapshot. Three months of home readings, weights and oxygen levels recorded by the same nursing team is a trend — and trends are what reveal early heart failure, worsening lung function, an infection brewing or a gradual decline that no one has quite named yet.
Because our nurses see you in your own home, they also see the things a clinic never does: the loose rug, the empty fridge, the medicines still in the packet, the walk to the bathroom that has become risky at night.
Assessments are frequently the entry point to other DVA nursing support, including medication management, chronic disease management and continence care.

Eligibility
Who can access DVA health assessments and monitoring
If you hold a DVA card and your health needs clinical oversight at home, an assessment is usually approved with a referral.
- DVA Gold Card holders with an assessed nursing need
- DVA White Card holders for accepted conditions
- Veterans referred by a GP, hospital or treating specialist
- Veterans whose family have noticed a decline at home
- Veterans with several conditions needing regular observations
- Veterans at risk of falls, malnutrition or skin breakdown
