Medication support our DVA nurses provide
Every visit is documented, so your GP, pharmacist and family can see what was given, what was missed and what needs review.
Webster and blister pack support
Pack ordering with your pharmacy, weekly checks, prompting and supervision so doses are not missed or doubled.
Medication administration
Nurses administer oral medicines, patches, eye and ear drops, inhalers, creams and PRN medicines as prescribed.
Injections and insulin
Subcutaneous and intramuscular injections, insulin administration, blood glucose monitoring and safe sharps disposal.
Medication reconciliation
New prescriptions and hospital discharge changes checked against what is at home, with ceased medicines removed.
Side effect and risk monitoring
Observations, falls and confusion risk, blood pressure and pain review, with early escalation to your GP.
Education for you and your carer
Clear explanations of what each medicine does, plus training so partners and carers feel confident between visits.
Why it matters
Safer medicines mean fewer hospital admissions
Many veterans manage several medicines a day, often prescribed by different doctors. Add changing eyesight, dexterity or memory and the risk of a missed dose, a double dose or an interaction rises quickly — and medication-related harm is one of the most common reasons older Australians end up back in hospital.
Our nurses reduce that risk with structure: one accurate medication list, a dose administration aid that is checked in person, documented administration, and a direct line to your GP and pharmacist whenever something changes.
Medication support pairs naturally with chronic disease management and health assessments and monitoring, so your whole clinical picture is reviewed at the same visit.
Eligibility
Who can access DVA medication management
If you hold a DVA card and need nursing help with medicines, support is usually approved with a referral.
- DVA Gold Card holders with an assessed nursing need
- DVA White Card holders for accepted conditions
- Veterans referred by a GP, hospital or treating specialist
- Veterans returning home with a changed medication list
- Veterans managing diabetes, anticoagulants or complex regimes
