Each visit combines clinical monitoring with practical coaching, so small changes are caught early and daily routines stay realistic.

    Blood glucose monitoring, insulin administration, foot and skin checks, diet and hypoglycaemia education.

    Blood pressure, pulse, weight and fluid monitoring, oedema checks and salt and fluid guidance.

    Oxygen saturation checks, inhaler and nebuliser technique, breathlessness pacing and action-plan review.

    Pain scoring, analgesia effectiveness, falls-risk screening and referral for equipment or therapy.

Why it matters

Early monitoring keeps veterans out of hospital

Most chronic condition emergencies build slowly. A creeping weight gain, an oxygen level drifting down, a blood sugar pattern trending high, a wound that stops improving — each one is easy to miss at home and obvious to a nurse who sees you regularly and records the numbers.

That is the value of structured monitoring: your plan has clear thresholds, and when a reading crosses one, your nurse contacts your GP the same day rather than waiting for the next appointment. Many veterans avoid an admission simply because treatment was adjusted a week earlier.

Chronic disease management works best alongside medication management and health assessments and monitoring, so one visit covers your medicines, observations and overall wellbeing.

Eligibility

Who can access DVA chronic disease management

If you hold a DVA card and live with a long-term condition that needs nursing oversight, support is usually approved with a referral.

  • DVA Gold Card holders with an assessed nursing need
  • DVA White Card holders for accepted conditions
  • Veterans referred by a GP, hospital or treating specialist
  • Veterans managing diabetes, heart, lung or kidney conditions
  • Veterans with several conditions and multiple treating doctors
  • Veterans recently discharged with a new treatment plan
