Why it matters
Most readmissions happen in the first 30 days
Hospital discharge moves quickly. Medicines have changed, a wound needs regular dressing, a walking aid is new, appetite and strength are low, and the specialist review is three weeks away. It is a lot to manage alone — and it is why so many older Australians and veterans end up back in hospital within a month.
Structured nursing closes that gap. One accurate medication list, wounds reviewed by a nurse rather than guessed at, observations recorded against a baseline, and a direct line to your GP the moment something looks wrong.
Post-hospital care commonly runs alongside wound care and medication management, and steps down into health assessments and monitoring once you are stable.
Eligibility
Who can access DVA post-hospital nursing support
If you hold a DVA card and are coming home from hospital with a nursing need, support is usually approved on referral.
- DVA Gold Card holders with an assessed nursing need
- DVA White Card holders for accepted conditions
- Veterans referred by a hospital discharge planner or GP
- Veterans home after surgery, a fall or an acute illness
- Veterans with a surgical wound, drain or new dressing regime
- Veterans whose medication list changed during admission

4. Recovery and step-down
Visits taper as you improve, then transition to ongoing nursing or in-home support if needed.
