From a simple skin tear to a complex non-healing ulcer, your nurse follows an evidence-based wound management plan and documents progress at every visit.
Chronic leg and foot ulcers
Venous, arterial and diabetic foot ulcers with compression therapy, offloading advice and regular healing reviews.
Pressure injuries
Staged pressure injury care, pressure-relief and repositioning plans, plus skin integrity checks to stop new wounds forming.
Surgical and post-operative wounds
Suture and staple removal, drain site care and infection monitoring after joint replacements, cardiac and general surgery.
Skin tears and trauma wounds
Fragile ageing skin, skin tears, lacerations and burns dressed with products chosen to protect surrounding skin.
Complex and non-healing wounds
Advanced dressings, negative pressure wound therapy support and escalation to a wound specialist when healing stalls.
Documentation and reporting
Wound measurements, photographs and clinical notes shared with your GP and DVA so your treatment stays coordinated.
How a visit works
Assess, dress, review — the same nurses each time
Your first visit is a full wound assessment: wound type, size, depth, exudate, pain and the condition of the surrounding skin, along with the factors slowing healing such as circulation, nutrition, diabetes or mobility. Your nurse then writes a wound management plan and agrees a dressing schedule with you and your GP.
At every visit the wound is cleaned and re-dressed using aseptic technique, measured and documented, and any sign of infection is acted on immediately. Because we roster consistent nurses, small changes get noticed early — which is exactly what shortens healing time.
Wound care often sits alongside medication management and post-hospital nursing support, so one coordinated plan covers everything you need at home.
Eligibility
Who can access DVA wound care at home
If you hold a DVA card and have a clinical need for nursing, wound care is usually approved with a referral.
- DVA Gold Card holders with an assessed nursing need
- DVA White Card holders for accepted conditions
- Veterans referred by a GP, hospital or treating specialist
- Veterans discharged home after surgery or a hospital admission
- Partners and carers needing wound care support at the same address
